This was an SHVC board with vertical lines. Originally I suspected PPU1, and the CPU was my second guess. Neither were correct. Turned out to be PPU2 creating some visual artifacts.
All tests passed on the burn in cart and there were only mild visual issues in the test sequence, a tougher one to see from the test cart alone. But playing a game it was more obvious.
